HealthReach Community Health Centers is a group of twelve Federally Qualified Health Centers located across Central and Western Maine. A private non-profit with a 47-year history, HealthReach is funded by patient fees, grants, private donations, and by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS). HealthReach provides primary care services for the whole family. In 2021, HealthReach served over 26,000 Maine residents and visitors – regardless of insurance status or ability to pay. We strive to be the provider, employer, and healthcare partner of choice in each of the communities we serve.

Dedicated clinicians deliver high-quality medical and behavioral healthcare to our patients. All of our practices have achieved National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognition. The PCMH is a model of care in which the HealthReach clinical team partners with you as the patient to determine and meet your healthcare goals. Your care is centered on you as the patient. Our process ensures safe, comprehensive, well coordinated, accessible, and high-quality care for all of your medical needs.
Affordable
HealthReach services are available to everyone, regardless of ability to pay. To ensure access for everyone, HealthReach accepts Medicare, MaineCare, and major insurance providers. In addition, an Affordable Care Program (sliding fee scale) is available to both uninsured and underinsured residents. Staff are available who can help you apply to programs (including the Health Insurance Marketplace) that assist with the cost of healthcare services and medications.
Close to Home
HealthReach serves families from 9 of Maine’s 16 counties - Androscoggin, Franklin, Kennebec, Knox, Lincoln, Oxford, Sagadahoc, Somerset, and Waldo. In 2021 we saw over 87,500 patient visits. We work closely with four hospitals located in our service areas: Franklin Memorial Hospital (Farmington), MaineGeneral Medical Center (Augusta), Redington-Fairview General Hospital (Skowhegan), and Stephens Memorial Hospital (Norway). We also collaborate with local organizations, state agencies, and individuals who deliver crisis, health, psychosocial, education, social service, and other complementary supporting services to area residents.
